Access logs record who has seen a record and when, enabling the quick detection of unusual access rather than post mortem. We also have Business Associate Agreements with all of our clients that specify what we are responsible for under federal law, and what we can provide documentation of during an audit.
Handling detailed patient information in denials, appeals and payment posting means we have structured our workflows with minimal exposure. Staff only see what they need to see as they work and we clean up records on a prescribed retention schedule and not indefinitely. If there were to be a suspected breach of security, our plan is to immediately contain the breach, notify within the timeframe allowed by HIPAA, and thoroughly review to seal the leak.
